Your email address
Your social security number
Your snail mail address
Your home phone number and work number and if appropriate to receive calls there
Tell me the program you are using and the version (for example, word...95, 97, 2000; or wordperfect, or whatever program you use)
How long you have used a computer and the Internet
Do you give your permission for me to post your grades over the Internet (it would be password protected and only accessed by the person for their grade)
