Read These Instructions Carefully Before
Proceeding
These instructions are
provided as a guide to assist you in properly completing your personal
advisement enabling our faculty to assist in your future
endeavors in the criminal justice field.
Copies of the following forms must be on file in our office
before you receive a
form from us authorizing you to
register. You may bring your
originals at the time you submit your
application to our office. We will
copy and return the originals to you at that
time.
Copies needed:
- Notarized (State Issued) Birth Certificate
- Driver's License
- High School Diploma or G.E.D./or High
School Completion Transcript (GED
applicants must have 12 college hours)
- All college or university transcripts
(Official Transcripts will be required from the Admissions Office.)
- All Military DD-214 (Honorable)
- Current color photo
Fingerprints
must be processed Fingerprinting
services are available on the Odessa College Campus in room ET 202 (2nd
floor of the Electronics Technology building) For more information and to
schedule an appointment call 432-889-0672. You may use this service or have
fingerprinting done at a site of your choosing.
Your Personal History Statement (Academy
Application) must be submitted. This may be done online or manually - typed or
printed legibly in ink.
Answer all the questions to the best of your
ability.
If a question is not applicable to you, enter N/A in the space
provided.
Avoid errors by reading the directions carefully before making any
entries on the form. Be sure your information is correct and in proper
sequence before you begin.
If there is insufficient space on the form for you to include all
information required, attach extra sheets to the Statement. Be sure to
reference the relevant section and question number before continuing your
answer.
An accurate and complete form will help expedite your application.
